Home Safety & Support Program Application
As a way to give back to the community, we have created the Home Safety & Support Program in order to provide essential home maintenance and safety services at no cost to families caring for individuals affected by dementia, Alzheimer’s disease, Parkinson’s disease, and other life affecting conditions.
The program is designed to support families who demonstrate financial need and are actively engaged with the nonprofit, with the goal of improving safety, functionality, and overall quality of life within the home environment.

Scope of Services
Services may include, but are not limited to:
• Installation of grab bars and safety supports
• Trip hazard removal (loose flooring, thresholds, transitions)
• Minor accessibility improvements
• Door, lock, and basic access repairs
• General home safety enhancements
Exclusions
To maintain program sustainability, the following are excluded:
• Major renovations or remodeling
• Cosmetic or non-essential upgrades
• Emergency or after-hours services
• Specialized or highly technical work outside standard handyman scope
All services will be performed in accordance with program guidelines and available capacity.
Program Capacity & Limits
To ensure consistency and long-term sustainability:
• Up to two (2) service requests per month
• Each project limited to approximately:
o 4 labor hours
o $200 in materials
Additional Guidelines
• Limit of one service per household every 6–12 months
• East Island will source and purchase materials up to the program cap
• Projects exceeding limits may require:
o Scope adjustment
o Donor sponsorship (if available)
Capacity may be adjusted over time based on demand and available resources.
Scheduling & Execution
• Services will be performed during standard business hours
• Work may be completed during designated “community service days” or integrated
into existing schedules
• A basic work order and service agreement will be completed for each project
Liability & Insurance
• East Island LLC is fully licensed and insured
• All work will be performed with the same professionalism and care as paid services
Important Clarifications
• East Island operates as an independent contractor
• The Center for Memory & Movement bears no liability for work performed
• Clients will be required to sign a service acknowledgment and liability waiver prior to project commencement
